Are you living with a long term condition? If so, now is a chance to have your say on the future of the services provided.
Lancashire Care NHS Foundation Trust is working with partner organisations to improve the care provided to people living with long term conditions to enable them to live as active and independent life as possible.
Long term conditions can include a range of health issues such as diabetes and Chronic Obstructive Pulmonary Disease (COPD). The survey is being run by BMG, an independent research company, and all responses will remain confidential.
The trust, along with partner organisations including local Clinical Commissioning Groups (CCGs), Lancashire Teaching Hospitals and Healthwatch, held a public event in Autumn 2013 to explain the work that will be taking place to further support people with long term conditions and providing an opportunity for both patients and carers to express their views. The feedback from the survey will be added to comments from the event and will help to inform future plans and service improvements.
